Why should I and why should I not be afraid of the Coronavirus pandemic?

I will try today through a series of questions and answers to clarify some things that I hear concern people, and have not been answered satisfactorily yet, since no one discusses them much.

Let me clarify before I begin, that I am not an infectious disease specialist but rather someone who has done research on the molecular genetics of viruses and the biology of their transmission. So if somewhere what you read contradicts what others say, especially me or the Ministry of Health, listen to them. Also, if someone more specialized finds something wrong, please correct me.

1. Finally, when and how does the transmission of a virus end?

The answer is, never. Viruses never disappear completely. They are always circulating among us. The problem is that they circulate at a low - normal frequency.

2. What can increase or decrease the circulation of a virus?

When a virus, as it happens recently, is found in "virgin" soil, that is, among many people who are not immune to it, it tends to spread quickly! In just a few days it can infect thousands to millions of people.

On the contrary, when it is found in humans, many of whom have been in contact with it in the past and have therefore developed immunity, its spread is limited. This is what normally happens to all the other viruses we know.

Another key way of limiting transmission is through vaccination, which has essentially the same effect: creating people who are immune to the virus.

Finally, a virus, especially if it belongs to the virus transmitted by droplets such as the coronavirus, can put the environment: the drier and warmer a place is, the less a virus can withstand outside the human body (e.g. x. on a table, a glass or a knob).

3. So we should seek to infect as many as possible, right?

Indeed, every society should aim to infect 60-70% of its members in the medium term. When this percentage is reached (by infection or by vaccination) the life of the virus becomes very difficult! What we call "herd immunity" is achieved, which practically means that the virus statistically does not easily find "where to step". On the contrary, it often falls on people who are immune to it, thus inactivating it and destroying it.

It is therefore a given that in the end most of the population will be infected in one way or another. The big question, however, is WHICH GROUPS will be infected, until herd immunity is acquired.

4. What happens to anyone who becomes infected?

The vast majority do not even understand it. It contains the virus and simply transmits it, without showing the slightest symptom. Others, much less often, will develop mild symptoms, similar to those of a cold or flu (fever, runny nose, cough). Nor do they have any cause for concern. A few will get more seriously ill (they will develop Pneumonia for example). But they, too, with the right care in an organized hospital, will most of them get well. Only a few of them will eventually end up (usually due to the concomitant presence of serious heart and respiratory problems or other conditions such as diabetes, cancer or other conditions that lead to suppression of the immune system).

5. Why then all this doom? Since sooner or later almost all of us will be infected - and for most it will mean nothing - why so many measures?

The measures do not concern how many will eventually be infected. They concern who and when will be infected! In every society there are groups that are most at risk (mainly the elderly and those suffering from diseases like the ones I mentioned above). These groups, under normal conditions, are protected a) by the immunity of the herd (which we do not yet have for the Coronavirus) and b) by vaccination (which is also not yet available, for the specific virus). So our goal should be to develop herd immunity slowly, trying to involve as few people from vulnerable groups as possible.

6. Why slow and not fast? Why not do what the United Kingdom does, say? Why not say "let whoever is infected get infected and finish. Anyway, very few will eventually die from it!"

The answer is given by statistics, in combination with the capabilities of the health systems of each country. If we leave the spread uncontrollable, we may have 1000 people in high-risk groups this month, who need special care (ICU hospitalization, ventilator assistance, etc.). Our system does NOT have so many ICU beds. So some will inevitably be left out. Lives will be lost - even a few - but otherwise they would have been saved. If on the contrary these 1000 cases occur gradually within a period of 10 months (until, say, next December) then our system will treat them all in the best possible way. Only a few will die! We will have saved the lives of loved ones!

In addition, if we manage to extend the transmission time, we give time to those who are trying to develop the vaccine against this particular virus. Once on the market, the number of cases that will require hospitalization may not reach a thousand, but remain at 500 or 200 (generally we have not yet convinced the people of vulnerable groups to be vaccinated en masse. We still have a way to go to this society direction).

7. So what is the final conclusion?

The game has nothing to do mainly with HOW MUCH they will eventually be infected. Most of us will be infected! It has to do with WHO (we try to exclude as many of the vulnerable groups as possible) and NEVER get infected (we try to give time to the weather, the health system and the vaccine, "spreading" the infection as much as possible over time of the population, which will come sooner or later. But it is important that it comes sooner rather than later!)

8. And what should we do about it?

a) We stay home b) If we must go out we take measures: we avoid contact with people and objects that others have touched, we disinfect hands and surfaces c) if we have symptoms, we isolate ourselves from others. We do not go to work, we do not go shopping, we take care not to scatter droplets with our cough or sneeze.

Within a few weeks, herd immunity will slowly begin to build up. The coming of summer will also help. But until the vaccine comes out, until 60-70% of the population has been infected (which may not have happened by next Autumn) ATTENTION to the vulnerable groups! Only when we vaccinate them all will we be able to sleep in complete peace as a society!

9. What about churches?

The government decided to close cafes, restaurants, theaters, shopping malls. Generally any enclosed space where many people gather. He has not yet decided to do the same with the churches.

My opinion is that he will do it soon. If he does not issue an explicit ban, the recommendation already made by the Prime Minister applies: "the same applies to places of worship as it does to all other enclosed spaces: avoid large gatherings". But you are big kids and you do not need others to tell you what to do! You now have all the information you need. DO NOT go to church this Easter.

Churches now seem to be the only places where mass contamination can - and will - occur. Even if it is decided to hold the church in open spaces, you will still not be able to avoid the crowds.
